Since thymidine-sparing, NRTI-based regimens are associated with fewer instances of lipoatrophy and hyperlipidemia than thymidine-containing, NRTI-based regimens, switching to a thymidine-sparing, fixed-dose combination of NRTIs is feasible and associated with no unexpected adverse events, including those affecting renal function and bone mineral density. - The Role of Nucleoside Analogs in the Development of Insulin Resistance (July 24, 2007)
NRTIs to date have been thought to contribute to insulin resistance mainly via induction of lipoatrophy. The authors of this paper put forth another potential mechanism based on their findings: that is, zidovudine (AZT, Retrovir)-mediated mitochondrial toxicity in skeletal muscle, adipocytes and hepatocytes.
