Recommendation from the U.S. Centers for Disease Control and Prevention

Combination therapy (AI)
Peginterferon alfa-2b (1.5 mcg/kg body weight) SQ weekly; or Peginterferon alfa-2a (180 mcg) SQ weekly
and
Ribavirin PO (weight-based dosing: if <75 mg, 400 mg in a.m. and 600 mg in p.m.; if >75 kg, 600 mg BID)
