1st line ART in resource limited settings

QUESTION:

CAN WE TREAT WITH TENOFOVIR+EMTRICITABINE + NEVIRAPINE AS FIRST LINE IN RESOURCE LIMITED SETTINGS(NVP in place of EFA)

RESPONSE FROM:

Hello and thank you for your post.

I'd see no reason why one couldn't substitute nevirapine for efavirenz in resource-limited locations. It's reasonable to remember (though in practical terms less relevant for such places) that there's an increased risk of liver toxicity when nevirapine is initiated in persons with higher CD4 cell counts (>250 for men, >400 for women).

It's also relevant to remember that it's recommended to asses kidney function before and periodically during treatment with tenofovir.

    I hope this helps,
