Unusual ARV regimen

QUESTION:

A patient recently presented to our practice after relocating to the area. He is on a rather unusual ARV regimen which includes Epivir 150mg he takes every other day / 2 weeks on then 2 weeks off, Reyataz 150mg every other day, Viramune 200 mg every other day and Norvir 100mg every other day. The patient denies any history of medication problems. There are no renal or or hepatic issues.He has been maintained on this regimen for approximately 2.5 years. He denies being in any clinical trial. His CD4 count range is 177-198 with a viral load of < 75 copies. There are no available genotype results. I am concerned about maintaining this regimen due to dosing and increased resistance potential. Are you familiar with this type of dosing regimen and any suggestions as to modifications?

    Makes no sense. It would be interesting to know what his viral load is off of HIV meds or prior to their initiation.

    I would recommend using doses of HIV meds that have been found effective in clinical studies. Continued use of this regimen can risk resistance if his virus pops out of the episodic antiretroviral barrage.
