circumcision not the solution

Posted: Apr 22, 2008

QUESTION:

I do not agree with the idea of circumcision, it is, in my opinion, too premature to conclude that uncircumcised men have an increased risk of becoming infected with HIV and other STD's. Is it only a speculation that because it was done in a HIV vaccine trials, changes the immune system in the body to make the uncircumcised male to be at a risk to contract HIV or other STD? Was or are there any reports of uncircumcised males that did not get vaccinated, comparing that to the circumcised men, showing that still make them vulnerable to contract the infection? Is it because the substance produced in the foreskin that makes the man weak for immunity?Or it is largely due to the lack of hygienic practices? I think that educating the natural man hygiene to practices. And educate the general public there's nothing wrong being uncircumcised unless it is of pathological conditions such as phimosis that requires medical attention.

RESPONSE FROM:

There is nothing wrong with being circumcised and no man should feel ashamed of his foreskin or lack thereof. The studies (not the use of the plural as there are randomized studies of circumcision vs not that demonstrate clear advantages of the procedure) find that in Africa having a foreskin increases the risk of acquiring HIV. In the vaccine trial conducted outside of Africa, circumcision largely negated the risk adenovirus immunity carried among those vaccinated. Other data have also demonstrated links between lack of circumcision and other STDs.

The reasons are not clear but one can imagine that the foreskin may provide an environment (even among the fastidious) that can lead to the presence of a concentration of immune cells that can be vulnerable to HIV infection.

It is premature to recommend circumcision to all men but for those who are like those studied in Africa, this should be an option that remains on the prevention table.
