QUESTION:

WHO guideline 2006 mention the possibility of fixing the dose of d4T at 30mg irrespective of body weight to reduce toxicity. Can I apply to my clients?

    Thanks for your post.

    There is a small but important dataset addressing your question. Overall, the reduction in dose of d4T to 30 mg (from 40) in first-line treatment is associated with less toxicity and apparently similar antiviral effectiveness.

    While this is an improvement, current WHO guidelines also relegate d4T to a less than preferred status, that is to say that if other regimens are available (including AZT or tenofovir-based regimens), that they should be used first.
