AZT and lipoatrophy/lipodystrophy

QUESTION:

How significant is lipoatroph/ lipodystrophy associated with AZT because in our resource limited region we substitute d4T with AZT when this happens?


  

RESPONSE FROM:   

    While fat wasting during AZT treatment is well documented this seems to happen more rarely and at a slower pace than that seen with d4T.

    I think in your situation, changing from d4T to AZT when fat wasting is noticed makes sense. Tenofovir and abacavir, when they become more widely available, will be even better alternatives.
