CPK elevations and options

QUESTION:

Pt doing well CD4 at 450 vl undectable. On Truvada, Reyataz, Norvir increase CPK s to >1000 stop drugs for a week and level decreases what are my options highly experienced pt...


  

RESPONSE FROM:   

    CPK elevations can be due to a number of factors including medications, exercise and substance abuse. None of the HIV meds you list is commonly associated with increased CPK so I would restart the meds and see if the CPK rises again. If it does then it will take some detective work to find out which did it (look to past tolerance of these meds or again restart one by one).

    I would also advise the patient to avoid any strenuous exercise for a while and look to make certain no other prescription or alternative medications could be related to this lab abnormality.
