First line without facial wasting

QUESTION:

Lately I have had a few newly diagnosed patients who seem reluctant to begin therapy for fear of facial wasting. What regimen would you say is the least apt to cause this?


  

RESPONSE FROM:   

    This is a very common concern that I hear often. I think that starting with a regimen that includes either tenofovir or abacavir in place of either AZT or d4T should avoid this complication. Studies that have switched patients from d4T or AZT to abacavir or tenofovir have found improvements in fat wasting.

    The NNRTIs and FTC/3TC ae not associated with facial fat wasting.
