PCP and Toxo coinfection

QUESTION:

What are your recommendations on treating patients with Toxo who are also coinfected with PCP? Would IV Bactrim alone suffice?

    Thank you for your question.

    In our experience, we'd likely try to treat Toxoplasmic encephalitis with a first-line prefered agent, like pyrimethamine/folinic acid (typically 6 weeks), then transition to suppressive therapy with TMP/SMX. TMP/SMX might or should work, but there isn't sufficient experience to make this a strong recommendation. -BY
