QUESTION:

Please guide me as to which patients of ocular toxoplasmosis should be tested for HIV and can you please provide guidance on the treatment?

RESPONSE FROM:   

    Hello and thanks for posting. It makes sense to routinely check patients with ocular toxoplasmosis for the presence of HIV infection. Ocular toxoplamosis may be treated with pyrimethamine + sulfadiazine or pyrimethamine + clindamycin. Patients should also receive folinic acid with either regimen. In an immunocompetent adult, treatment is usually given for 2 to 4 weeks. If the patient has AIDS, then the treatment would need to be extended (or changed to secondary prophylaxis after recovery) depending on the clinical course and immune recovery from HAART.
