increase viral loas

QUESTION:

i have been on treatment since 2004 and have responded very well to the drugd but i have not become undetectable my last viral load was abobe 300o and cd4 above 500 but thre months before that i had a viral load of above 1000 and cd4 of above 600 . am i doing well or is my health failing though i do not feel sick but i am very woried about the results. oby from nigeria

    Dear Oby from Nigeria,

    I too am concerned about your viral load-- if you've been adherent to your medications and not otherwise ill, a repeated viral load above 400 is indicative of the potential evolution of drug resistance to your current regimen.

    I'm not entirely sure of the medical situation in Nigeria, but if this is your first drug regimen, it may be time for your doctor to consider switching to a second-line treatment regimen.
