osteoporosis

QUESTION:

hello sir. i have a child 12yr female on d4T+3TC+NVP since the last 2 years. had a minor fall as per mother and fractured both radius and ulna. i am suspecting osteoporosis due to HAART. how do i go about. what are the investigations. i do not think she can afford dexa scan. what is the managment. thank you. bunty


  

RESPONSE FROM:   

    At the very least, the child could start supplemention with calcium and vitamin D. If there is a nutritional deficiency this too would need correction. I do not think a change of antiretrovirals would improve things. Of course, adequate sun exposure is also important and children, especially females, who are often covered, such as with a burqa, may develop vitamin D deficiency.

    Alendronate is not recommended.
