CD4/viral load

QUESTION:

I hava seen patients with undetected viral load and and CD4 counts <10 cells/ml. my plan will be to booast this patient's immunity what best combinations can i offer. Can i switch from NVP to EFV since the recent guideline recommmends EFV.

    Thanks for your post.

    While it's not clear to me that a switch from a suppressive nevirapine regimen to efavirenz will result in a change in CD4 cell count rise, such a switch should be safe and generally well tolerated.

    If the issue is CD4 count increase, it's worthy to point out that recent clinical trials, including the ACTG 5142 study have shown statistically significantly greater increases in CD4 counts among patients that receive ritonavir-boosted PI regimens, compared with NNRTI-based regimens.
