QUESTION:

48 yr HIV+ patient CD4431 VIRAL LOAD <40 PLASMA LACTIC ACID >3.7 TREATMENT VIREAD 300MG NOCTE STOCRIN 600MG NOCTE LAMUVIDIDE 150MG BD.REPLACED ZERIT WITH VIREAD DID I DO THE CORRECT THING?What tablet is available in South Africa to take od?Dr.F.Booyens

    Dr. Booyens,

    Thank you for your post from RSA. In all likelihood, your patients hyperlactatemia was caused by the stavudine (d4T, Zerit). There is very good supporting evidence that the switch to tenofovir (Viread) should alleviate this condition, though if your patient is seriously ill or symptomatic, holding ARTs would be considered.

    All of the medications that the patient is currently taking (tenofovir, lamivudine and efavirenz) may be taken once daily (OD).
