QUESTION:

42yrs old female HIV+ since 1993.Was started on viracept and combivir which she took x one yr, self DC med over a yr ago with a vl of 1400. Now wants to get back on meds cd4 707 vl undetectable. Start med since she had been on before?

    Thanks for your post.

    It would be very reasonable to resume a previously successful regimen.

    That said, the regimen of ZDV/3TC (Combivir) and nelfinavir (Viracept) is dated and no longer part of treatment guidelines. Combivir remains an effective twice-daily option (especially since your patient tolerated this in the past) that is part of guidelines. Other once-daily nuke combos, such as Truvada or Epzicom are more commonly prescribed.

    Unboosted protease inhibitors have been replaced by more potent and better tolerated once-daily ritonavir-boosted options, like atazanavir (Reyataz) and fosamprenavir (Lexiva, Telzir). In our patients and in clinical trials, once-daily lopinavir/ritonavir isn't as well tolerated; it's pretty unusual for us to start patients on once-daily Kaletra.
