QUESTION:

my patient have aids + TB his resistance test :NRTI & NNRTIs (R);3TC&PIs (s) CD4% :6%; CD4: 126;Viral load:86000/ml he is on Rifampicin,INH Could we use PIs ? dose?

    Thanks for your post.

    There are a number of very serious drug-drug interactions between PIs and rifampin-- I wouldn't recommend their combined use.

    By contrast, if you have access to rifabutin, PIs can be used without as significant a risk of drug interactions.

    Clearly your patient needs to be on effective HAART, without rifabutin, your options would appear to be seriously limited until your patient can safely discontinue rifampin.
