HAART in liver failure

QUESTION:

I have an AIDS pt with liver failure due to alcohol, possibly also from HAART (AZT). He has stopped drinking and I am trying to get his T-cells above 100 so that he can be considered for a liver transplant study. Which medications do you think would be the safest? He has M184V and K103N mutations and chronically elevated INR


  

RESPONSE FROM:   

    Tenofovir shoudl not be a problem. I would use FTC as well as part of Truvada just to maintain the 184 and reduce viral fittness. I would go with a ritonavir boosted PI such as Kaletra here and keep my fingers crossed.
