Use of RIfampicin with Nevirapine combinations

QUESTION:

Rifampicin has been shown to reduce Nevirapine levels. However in resource limited setting most patients can only afford triple therapy combinations that contain NVP. Some doctors have sggested a patient on NVP can take Rifampicin with no dose adjustments? What is your opinion?


  

RESPONSE FROM:   

    The need to treat both TB and HIV is very common in many parts of the world.

    I would be very wary of using rifampin with nevirapine, especially while initiating treatment (because of the potential drug-drug interaction).

    There are studies that suggest that the combination can be used with similar efficacy- a very encouraging data point. We've suggested the substitution of nevirapine with 800 mg efavirenz when dosed with rifampin. BY
